1. The Basics
Full Name: Perhaps the most straightforward part of the application. This is the kind of thing that would go in your character's official signature. First, middle, and last name along with any titles, maiden name, whatever. Eg. Sir Sidney Longfellow Baindrige Jr. Esquire, M.D.
In case you missed it, this is actually a repeat question. Your character's name (at least first and last) should be in huge type at the very top of the application.
Call Sign: Any character that is a current or former aviator (i.e. Viper Pilot, Raptor Pilot, or Raptor ECO) will have/had a call sign. So long as it's not claimed
here then it's up for grabs.
Call signs range from the serious to the silly. Most of them have some kind of meaning behind them and were given to the aviator by others, often back in flight school or shortly thereafter.
The call sign is what the character will be referred to over the wireless rather than by name, and often while out of the cockpit others will call him by the call sign as a regular nickname.
Non-aviators do not have call signs (at least not in the sense that aviators do) so skip this section because it does not apply.
Nickname(s)/Alias(es): Is your character known by anything other than their given name or the above call sign? Put it down here then. Include: Abbreviations of the full name (Sidney -> Sid), variations of their name (Samuel -> Sammy), initials (Tyler James -> T.J.), completely new nicknames (Drake -> Drunken Bastard), that name you took after fleeing your homeworld when you murdered that guy (Roger -> Bob McFakename), and the name you gave to your captors when they were interrogating you (Onas -> Phuk Yu).
Gender: Hopefully there is a very easy answer to this question.
Sexual Orientation: Most people will be heterosexual but other alternative sexual preferences are available. The BSG universe for the most part is much more tolerant and accepting of such things than modern day.
Age: You'd think this one would be easy too, but especially when making a military character there is a lot of math that goes into when they went to school, when they joined up, when that got such-and-such promotion, when they got transferred to ship-x from ship-y while Joe Bag-of-Donuts was also on it. If you're having trouble getting a timeline together then just ask for help.
Birthday: This matters little, but we do keep track of it in case anyone actually wants to celebrate or casually mention it IC. We also update character ages periodically. Maybe you need your character born at a special time of year (he was born during a blizzard in your history so it needs to be winter or whatever) but most won't require anything like that. In that case then just go crazy and pick a random day that you like. If you can figure out the birthyear math then good on you. It should help with the history.

here. Having the same birthday as another PC can be interesting but if four people already have the same day then try and be unique and pick something else.
As a final note, keep in mind where we are in game time. If you make your birthday in early November and your character gets approved while the game is in late October then your character is going to have their birthday nearly immediately and age up a year.
Birthplace: This is actually a two part question and is potentially more difficult than it appears on the surface. For many, you will have been born, raised, and lived your whole life on one Colony. Others might have been born someplace while their parents were on vacation, went home, then went to school someplace else, moved and worked on yet another Colony, served hard time on a fifth colony, etc.
When it comes down to it, this is a citizenship question. Where were you born? Where are you registered to vote? Some people might have two (E.g. Walter Gamble was born on Canceron but his family moved to Caprica when he was about nine so his answer would be "Canceron (Caprica)").
Now that covers most but not all people. There are some outlying colonies of the
Twelve Colonies such as Troy and Pallas. There are also many space stations, asteroid bases, and people who might have just lived their entire lives aboard a ship and never had a home planet. In that case you'd put your first answer as where you were born and live. Your second answer would be the associated colony. For example if you were born on a mining Colony of Aerilon then you'd technically be Aerilonian. If your birthplace still lacks affiliation then just put "spacer" since you have no real home.
Occupation: Hopefully you've already got some idea of what you want your character to be. If you have no idea or you're still trying to nail down something specific then head to
this long ass thread. Some positions might already be filled but for the most part it's all open and ready for you to choose from. If you have some kind of specialty in mind or some other idea then feel free to present it to the staff for approval.
For civilian characters, occupation is a bit more straightforward and does not have to be nearly as specific as the military jobs. E.g. Cargo ship captain, fry cook, hair stylist, lion tamer, etc.
Rank/Title: Civilians have their own titles which should be easy enough (e.g. Captain Crunch, Sir Lancelot, Doctor Baltar, Chief Engineer Adams, etc.). When it comes to military characters, it once again gets a bit messy. Rank needs to correspond primarily to age and time in service but is also influenced to a lesser extent by your above military occupation, your pre-service education and experience, the needs of the service, your character's abilities and skills, and any incidents in the past.
The basic principle is to make the rank believable. First of all it has to fit your position (e.g. deckhands are not officers). Second it needs to fit your history (e.g. How could you have gotten five hundred men killed in a training accident and then immediately gotten promoted?). Third it needs to fit your age (e.g. You're not going to be a 20 year old admiral). More information on ranks and what is reasonable can be found
